These were: 1. The images of Figures 1 and 2 were published the wrong way around. The Figures’ legends were correct, however. 2. An incorrect sentence in the second paragraph on p. 191 should have been omitted. 3. There were errors in note 52 on p. 214. All of these mistakes have now been corrected in the online version of the article. The publishers apologise for the errors.
