[The natural history of advanced heart failure is progressive and eventually terminal. Patients diagnosed with end-stage heart failure exhibiting New York Heart Association (NYHA) Class IV heart failure symptoms and on optimal medical management should be considered for left ventricular assist device therapy (VAD). Escalation of goal-directed medical management, intolerance of neurohormonal antagonists, and increasing diuretic dosing are just a few triggers for referral for VAD therapy considerations. Absolute and relative contraindications are outlined in this chapter and should be collectively considered when determining candidacy for mechanical support therapy. This chapter will detail these important variables to assist the medical team in determining the treatment recommendation for advanced heart failure therapies.]
